Intramuscular Injection of the Combination of Hloperidol and Lorazepam in the Psychiatric Emergency



Abstract
The aute managements of severe disruptive behavior in psychiatric emergency setting consist of several methods such as calming talking. Seclusion, physical restraints. Pharmacotherapy. And electroconvulsive therapy. Many patients with severe
disruptive
behavior do not respond to non-psychopharmacological treatment. and so the pharmacotherapy is required frequently in clinical practice. Of the psychopharmacotherapy for control of psychotic agitation. There are some evidence that the combination
of
antipsychotics and benzodiazepines is better than are some evidence that the combination of antipsychotics and benzodiazepines is better than antipsychotics alone. benzodiazepines alone, and any other pharmacotherapeutic approaches. because of
the
rapid
onset of sedation. The safe pharmcological properties. And good compliance of maintenance therapy.
We tried to evaluate the efficacy of combination of intramuscular Haloperidol and Lorazepam. Comparing with intramuscular Haloperidol alone. The subjects consist of 40 patients (24 males and 16 females. Mean age ; 28.5¡¾11.5), and they wers
diagnosed as
12 schizophrenia and 28 bipolar disorder. Ail were assaultive. Aggressive. Violent. uncoperative. and extremely agitated state. We used the Visual Analog Scale for measuring the severity of agitation and the degree of sedation at every 30 minutes
for
total 180 minutes. We radomly selected the subjects and injected intramuscularly Haloperidol 5mg or Haloperidol 5mg-Lorazepam 4mg ar every 30 minutes till the subjects were calm down. We assessed the required dosage of Haloperidol for sedation.
The
time
onset of sedation. And the side effects.
We suggest that the combination of Haloperidol and Lorazepam is better than the Haloperidol alone for controlling psychotic agitation in psychiatric emergency because of 1) faster onset of sedation, 2) less required dosage of antipsychotics, and
3)
few
side effects of antipsychotics.
